- Combined Photo of recipient, donor and spouse
- Combined Photo of recipient and donor
- Combined Photo of recipient and spouse
- Relationship certificate from taluk Dhasildar
To get this first you have to take signature from your VAO (Village administrative Officer) and RI (Revenue inspector). Afterwards you have to forward the application into Dhasildar. At that time, you just explain about your problem to your Dhasildar.
- HLA test
- Consent letter by donor with Notary public affidavit
- Consent letter by recipient with Notary public affidavit
- Consent letter by donor’s spouse with Notary public affidavit
- Xerox copy of National ID card (aadhar card) recipient, donor and spouse.
Procedure to be carried:
- First authorization committee.
First we need to get the approval from DM office which is placed in Chennai kilpauk, that committee checks our originality of our documentation, an individual counseling will be taken between recipient, donor and donor’s spouse with the authorization committee. Once the committee approved your application means, you will get confirmation letter within seven to ten days.
- DM office theynampet committee.
This committee very recipient and donor’s medical fitness. After completion of this committee, you are ready to do renal transplantation and Wait for yours doctor confirmation.
To get medical fitness for surgery we have to take the fitness certificate from the following doctors:

After completion of all the tests you have to take fitness certificate from anesthetists Finally.
Anesthetists doctors will validate your all fitness tests results and all doctor’s opinion.
The above (anesthetists) evaluation will be done before one week from the date of surgery.
